Apply for Food and Nutrition Services?

If you think that you might be eligible for Food and Nutrition Services and want you apply, you need to:

- Call or go to the local department of social services for an application or ask them to mail you an application.

- Fill out the application as much as you can. Put your name and street address on the form and sign it.

- Return the application back to your local department of social services office.

- Have an interview. You can do your interview over the phone or you can have a friend or relative go to the interview in your place.

- Show the local office important papers such as pay stubs, receipts for rent or mortgage payments, utility bills, child or elder care bills, child support court orders and cancelled checks. Proof of identity and proof of income are also needed.

- After reviewing your application, the local office worker will tell you if you qualify and how much you will receive.

Apply for Family and Children's Medicaid?

You can come to DSS to apply or you can fill out the DMA-5063 Medicaid for Family and Children's Medicaid application, and mail it to Beaufort Co. DSS, P.O. Box 01358 Washington, NC 27889.

What will I need to complete my application?

- Pay stubs of the previous month if you, your spouse, or parents are working, Certified Birth Certificate of who is applying (if you do not have them DSS will help you get them).

- Two proofs of residence, (valid NC ID or drivers licenses with the correct mailing address and some type of mail such as an electric bill, water bill, or doctor's bill).

If any other information is needed a Caseworker from DSS will contact you.

Become a Foster Parent?

To be a foster parent, you must meet the following criteria:

- Be at least 21 years of age
- Married or single
- Have a stable home and income
- Complete all required training
- Committed to children

Make a Child Protective Services Report?

Beaufort County Department of Social Services holds the responsbility for accepting reports of abuse, neglect, and dependency of children that have been abused, neglected, or exploited.

To make a CPS report: Citizens of Beaufort County should report suspected allegations to the agency during working hours 7:30 am - 5:30 pm, Monday through Friday by calling 252-975-5500. The call will be directed to appropriate department. After hours, weekend, and holiday reports can be made by calling the Beaufort County Sheriff's Office/Communications at 252-946-0101 to reach a social worker on call.

Make an Adult Protective Services Report?

Beaufort County Department of Social Services hold the responsbility for accepting reports of abuse, neglect, and dependency of disabled adultsthat have been abused, neglected, or exploited.

Find out if I am eligible for CAP/DA?

To receive CAP/DA services, you must live in Beaufort County and be:

18 years or older
Eligible for Medicaid
Approved for Nursing Home Care
Be at risk of Nursing Home Placement
Maintained safely in the home within the CAP/DA cost limit.

Find out if I am elibigle for CAP/C?

To receive CAP/C services, the child must live in Beaufort County, be up to 18 years of age, and has severe medicial needs.

Apply for Work First Family Assistance?

How to Apply:  Applications are taken during the hours of 7:30 am – 5:30 pm Monday thru Friday.

What you will need: 

- Birth certificates

- Social Security cards

- Previous month’s pay stubs

- Health Insurance cards

- Most recent bank statement

- Life insurance policies

- The names, addresses, and date of birth of the absent parent.

Apply for Medical Transportation?


How to Apply:  Come to the agency during the hours of 7:30 am and 5:30 pm Monday thru Friday. You may also request assistance via the Medical Transportation Phone Line at 252-940-6102.

Apply for Crisis Intervention Program (CIP)?

How to Apply:

- Applications are taken during the hours of 7:30 am and 5:30 pm Monday thru Friday.

What You Will Need: 

- Past due heating or cooling bill.

Apply for LIEAP (Low Income Energy Assistance Program)
  • There will be no automatic payments made in February 2012
     
  • Applications taken during December 1, 2011 through January 31, 2012 will target: households where ALL members are age 60 or older and households where at least one member is Disabled AND receiving services through the Adult and Aging Program

  • All approved payments will be made directly to the Energy Providers

  • If funds remain available, all other households may apply beginning February 1, 2012 through March 31, 2012
Apply for Adult Medicaid?

If you receive Supplemental Security Income (SSI), you are automatically covered for North Carolina Medicaid.  You do not need to apply for Medicaid.  However, if you need assistance with nursing home cost of care, you would have to update your Medicaid record. You may apply at the County Department of Social Services (DSS) in the county where you live.

How to Apply: If possible, take the items listed below with you when you go to the DSS office to apply.

  • Certified birth certificates or other proof of citizenship/alien status for each individual applying for Medicaid/NCHC

  • Identity documents for each individual applying for Medicaid/NCHC

  • Social security cards, social security numbers, or proof that you have made an application for a number from the Social Security Office, for each individual applying for Medicaid or NCHC

  • A copy of all pay stubs for the last 3 months

  • Copies of all medical or life insurance policies

  • A list of all cars, trucks, motorcycles, boats, etc. you or anyone in your household own, including the year, make, model, and vehicle identification number (VIN) for each item

  • Most recent bank statements

  • A list of all real property you own

  • Current financial statements/award letters from other sources of income, such as social security, retirement benefits, pensions, veteran benefits, and child support.

If you are unable to go to a DSS office, you may also print and mail completed Medicaid Applications to your local DSS office, or you may call and ask that an application be mailed to you.  Faxed applications and applications received over the internet are not acceptable.  Some health centers and hospitals have application forms available and may be able to assist you in completing them.
